School Self Assessment & Evaluation Report
Every 2008–09 grantee must participate in two self evaluation exercises. The first report is due no later than December 12, 2008; the second is due no later than May 22. TALC will review these reports and also assess schools using the same criteria following site visits to the schools to generate a Mid-Year and End-of-Year assessment reports.

Use of Funds Form
1. Twice in 2009 (on January 15 and June 19) the team leader or person authorized to do school accounting functions must complete and submit the TALC New Vision “Use of Funds” Excel auto-calculating form. When downloading the form, be sure to save the spreadsheet to your personal computer – do not attempt to fill it out online. You must list each receipt or invoice on a separate line, indicate which main grant budget category the expense falls under (see budget codes on the form), type in the name the vendor, give a brief description specifically tying the expense to your approved grant budget, and enter the amount in the “amount” column. Appropriate back-up must accompany the form; please attach copies of receipts and/or paid invoices as well as check stubs if available.
2. The team leader or authorized financial representative must sign the TALC New Vision “Use of Funds” form authorizing release of funds.
3. All “Use of Funds” reports are reviewed to verify copies of the receipts/invoices are attached, that the expenditure is grant compliant (see the TALC New Vision Guidelines & Procedures Handbook for rules and restrictions), is tied to your approved grant budget, and the budget shows sufficient funds are available.
4. Grantees should pay their vendors directly out of their school accounts and report their grant expenses semi-annually.
5. The deadlines for submitting “Use of Funds” reports are January 15, 2009 and June 19, 2009.

Receiving Stipends
1. Any individual requesting stipends must complete and submit a W-9 tax form to the TALC New Vision office. (A downloadable pdf is below.) Click on the W-9 pdf link and print the form. Fill out the top half of page one. Where the form asks for “Business Name” enter the name of your small school. Enter your home address (no P.O. Boxes allowed), your social security number, sign the form, and return it to TALC New Vision. No checks will be issued without a completed W-9 on file. If you move, a new W-9 is required.
2. Each individual must complete the TALC New Vision Excel Stipend Payment Request form (downloadable excel document below - be sure to save the document to your personal computer before using the form). Record dates, describe allowable activities indicating how the work specifically ties to your approved grant budget, indicate the number of hours for each date, and enter the approved rate. The excel spreadsheet will auto-calculate and auto-sum. Stipends will be paid quarterly in October and January. No stipends will be allowed in 2009.
3. Give your signed form to your small school team leader for an approval signature. If you are a team leader with an MPS school, you must submit your personal stipend request form to Marty Lexmond for an approval signature prior to sending to TALC New Vision. If you are not a team leader of an MPS school, be sure to follow whatever protocol your team set up to provide checks and balances. No individual may approve their own stipend request.
4. After appropriate approval, all stipend forms must be delivered to the TALC New Vision office or postmarked by the deadline dates.
5. Please submit one stipend payment request form per person. Do not put multiple individuals on the same form.
6. Stipends are paid quarterly. Stipend requests must be submitted quarterly as follows: by October 6 for July, August, September; by January 5 for October, November, December. Checks will be generated no later than the end of the month in October 2008 and January 2009.
7. All requests are reviewed to verify: the work is documented, grant compliant, and included in approved grant budget; the rates are appropriate; and there are sufficient funds available in the approved grant budget.
8. All checks will be mailed. There will be no exception to this rule.
9. Stipend requests dating back futher than the previous quarter will be denied. Do not wait until the end of the grant year to submit stipend requests.

Allowable Stipend Expenses
The purpose of stipends is to invest in professional development activities that support teachers in the development and early implementation of small high schools in Milwaukee. Stipends are intended to provide compensation for group planning sessions and professional development activities that take place after the regular school day or on a Saturday. Stipends are not to be viewed as supplemental pay or salary for individuals engaged in typical school activities that go beyond the regular school day.
Additionally, stipends will be paid only up to the amount approved in a grant budget. Stipend requests will not be fulfilled if the stipend budget line is exhausted.
Examples of non-allowable stipend expenses are:
1. Staff positions, salaries, including clerical/administrative support;
2. Attending conferences or for travel time;
3. Engaging in typical school activities;
4. Completing paperwork submitted to TALC New Vision.
Contracting With Consultants
Consultants must have an approved professional services contract. A copy of the contract must be submitted to TALC New Vision with the semi-annual “Use of Funds” report before any semi-annual payment can be released. If the consultant is working with Milwaukee Public Schools (MPS) students, please follow standard MPS protocols for consultants. Annual Demographics Report
All non-MPS grantees are required to submit an annual demographics report to TALC New Vision on or before July 15 for submission to the Bill & Melinda Gates Foundation (Marty Lexmond provides the data for the MPS grantees).
Fill out the demographics report according to the sample page and email to emcmanus@talcnewvision.org at TALC New Vision.

Contact Information Form
Each team is asked to annually submit contact information forms for each of the team’s (up to 6) core members. Completed forms can be faxed back or mailed to TALC New Vision.
